DMACC will no longer make name changes via phone calls or emails, by Program Change forms, or Admissions Applications. Names will remain unchanged until the appropriate documents have been received and we have a student signature on the request.
We encourage our students to either fill out one of the name change requests below or to contact Academic Records at 515-964-6396
